You are digging your middle irons, hybrids, and shorter woods into the ground. This is preventing you from completing a full finish.
Be certain to allow the club to release. This will allow you to move through the ball with a proper finish.
Put a golf tee into the Velcro of your glove so that it is pointing out, across the V formed with your thumb and index finger. Focus on allowing the tee to point down the target line as the club moves through the ball. This will then allow encourage the shaft of the club to point down the target line as well.
